Thesis abstract

Lidocaine is a local anesthetic used daily in anesthesia, particularly in continuous intravenous administration (CIVA) to reduce the use of opioids and their side effects. Although factors such as age, obesity, liver failure and cardiac output influence its pharmacokinetics (PK) and pharmacodynamics (PD), a single dosage is currently recommended by the French Society of Anesthesia and Intensive Care. Few studies have evaluated the PK of lidocaine in specific populations. Existing studies in pediatrics or the elderly are limited and not transferable to practice (small sample sizes, administration of a single bolus without CIVA). Three clinical studies have therefore been designed for this project: ElderLIDO (PK in elderly subjects to optimize dosage and limit opioid use), KidLIDO (PK in pediatrics to standardize and ensure safe administration), ILHEP (PD after liver resection). A meta-analysis of individual patient data (MIPD) using existing patient data in the literature will also be conducted to answer this question. However, despite international recommendations, the sharing of data from randomized controlled trials remains limited in practice. An interim study will therefore assess the transparency of anesthesia reviews and the reproducibility of published randomized controlled trials in order to promote open science. The MIPD on lidocaine in a special population (elderly, obese) will be carried out after this interim study.

The aim of this work is to develop a personalized lidocaine dosage algorithm.
